Document Submission Requirements:
To upload your claim documents:
- Enter your Claim ID found at the top right of your claim form.
- Enter your Email Address.
- Re-enter your Claim ID and Email Address.
- Select ADD DOCUMENT.
- Find the document saved on your device.
- Select your document and click Open.
- Repeat Steps 3 - 5 until all requested documents are added.
- Click BACK TO TOP.
- Check the box under Document Verification.
- Click SUBMIT.
ONLY FIVE DOCUMENTS MAY BE UPLOADED AT A TIME. IF YOU HAVE MORE THAN FIVE DOCUMENTS, PLEASE SUBMIT THE FIRST FIVE FOR UPLOAD AND REPEAT THE PROCESS UNTIL ALL FILES HAVE BEEN UPLOADED TO YOUR CLAIM.
Document Requirements:
Files:
- Must be in pdf, tif, png, or jpeg format.
- Must be greater than 1 byte, and less and 10 MB.
- Cannot be password protected.
File names:
- Can only contain letters, numbers, dashes, and underscores.
- Cannot exceed 100 characters.
Certain claims require original documents in order to process and therefore cannot be uploaded to the website. Please refer to your claim form to determine if your claim requires original documentation. If you are unsure, please
